Forms W-2/W-3 - $25 + $3 per individual form

All Forms 1099/1096 - $25 + $3 per individual form

Form 1098 - $25 + $3 per Individual form

Forms 940/941/944/943 - $35 per return

NC and Other State Forms - $25 per return for most returns – Other States could be higher
